If you were separated or retired from the Army, the form that contains the PEB findings is called "Physical Evaluation Bard (PEB) Proceedings," or a "DA 199," and you can view a sample of the form by clicking here. If you were separated or retired from the Air Force, the form is called "Findings and Recommended Disposition of USAF Physical Evaluation Board," or an "AF Form 356," and you can view a sample of the form by clicking here. If you were separated or retired from the Navy or the Marine Corps, the form that contains the PEB findings is called "Findings of the Physical Evaluation Bard Proceedings," and you can view a sample of the form by clicking here.
In addition, please be sure to provide us with your full name (including any alternate names, e.g., maiden name or previous married name), your social security number, home address, primary and alternative phone numbers, and email address (if any).
Within two weeks of receiving the copy of your PEB findings, you will be contacted by a member of class counsel's staff as to whether the lawsuit applies to you.
